Thanks Elaine.  I finally bit the bullet and called the insurance company. After an hour on hold, they were able to tell me they received and approved the request yesterday afternoon, but could not tell me what all was approved. Unbelievably,the surgeon's coordinator has still not returned my calls from yesterday and the day before.  Were it not for the fact that I do not have the time to start this process all over again, I would dump both practices and start again elsewhere.  I definitely have concerns that not everything is covered somehow, but I'm pretty stuck at this point.  I am contemplating gathering a few notes tonight and calling tomorrow with a request to talk to someone in charge.
